Sometimes, our young people and families just need a little extra boost to help them identify and access the resources they need to succeed in their schools, their work, their homes and their communities. At St. Aloysius, our team of case managers stand ready to assist with a wide variety of challenges based on an ongoing assessment of an individual needs.

Case Managers Can Make a Difference

At St. Aloysius our Case Mangers make a difference in the lives of our students and families by providing:

  • Assistance in achieving personal independence through effective management of basic needs
  • Development of daily skills
  • Coordination of treatment, such as therapy and medication management
  • Advocacy and outreach
  • Assistance with accessing natural and sustainable support systems in the community
  • Linkage to more formal, professional services and systems
  • Monitoring of symptoms and progress in treatment
  • Assistance during times of crisis when strong and stable supports are needed
  • Education and training that is tailored to your and your child’s specific needs, abilities and readiness
  • Treatment interventions that address symptoms, behaviors, thought process and other difficulties

 Requirements

  • We accept: Medicaid, Private Pay*, Aetna Better Health of Ohio, Meridian Health Plan, Paramount Advantage, United Health Care and Caresource
  • Parents/Guardians are required to be present during intake and treatment team meetings every 90 days

Working to Support Your Child’s Success

St. Aloysius wants to offer your son or daughter an alternative to formal therapy with Community Psychiatric Support Treatment, or CPST. With this case management service we can help your child’s treatment team get a real understanding of the unique challenges your child faces. We work hard to support your child’s success—at home, at school and in the community.

Case Managers Can Make a Difference

The Community Psychiatric Support Treatment offered at St. Aloysius provides our children and families with the informal supports that fit their needs. This case management service can be conducted in the home, at school or in community settings based on the needs of the client and family.  The goals of our CPST workers are as follows:

  • Skills building to develop positive adult and peer relationships and promote social independence
  • Educational assistance
  • Monitor service providers and other agencies to aid in moving the client toward adaptive and independent functioning
  • Linking the client and family with community resources that provide positive recreational outlets and support
  • Coordinating team and community members, agencies and providers in meeting the client’s mental health needs to promote success and independence
  • Assess client’s current and/or ongoing needs
  • Empowering the client to make positive choices and resist the negative influence of negative peer pressure
  • Advocacy for the client and family to increase involvement with community resources and teaching them to advocate for themselves
  • Crisis support, including development of a crisis plan with client’s child/family team
